The University of Wisconsin Medical Alumni Association Awards Committee invites you to nominate your colleagues and classmates for the 2010 Medical Alumni Association Awards. Awards include:

  • Emeritus Faculty Award 
  • Honorary Life Memberships
  • Medical Alumni Citation Award
  • Medical Alumni Distinguished Teaching Awards
  • Medical Alumni Service Award
  • Ralph Hawley Distinguished Service Award
  • Sigurd Sivertson Medical Education Award

How to Nominate

 

University of Wisconsin School of Medicine and Public Health alumni, faculty, staff and professional colleagues may submit nominations.

  • Write a letter stating the award for which you are submitting the nomination, and a detailed outline of the nominee’s qualifications

  • Include the nominee’s curriculum vitae, including current address and awardsphone number

  • If available, secondary letters or materials supporting the nomination
