UW Health’s Clinical Simulation Program (CSP) partners with departments across UW Health and the UW School of Medicine and Public Health to implement effective simulation education. Submit this application to request usage of the CSP’s resources. A CSP team member will review your request and get back to you as soon as possible.

Scheduling Process
- Complete this application
- Meet with curriculum planning CSP staff
- Complete the Curriculum Development Guide with CSP staff
- Walk through the simulation with CSP staff
- Implement/execute simulation education
- Debrief and evaluate (learners, instructors, environment, program)
- Revise and improve simulation with facilitators
