Faculty and Staff Equity and Diversity Award 2015-16

The University of Wisconsin School of Medicine and Public Health (UW SMPH) Faculty and Staff Equity and Diversity Committee is very pleased to announce the call for nominations for the Annual Faculty and Staff Equity and Diversity Awards, honoring the promotion of equity and improvement of diversity and climate by individuals at the School of Medicine and Public Health.

 

Three awards have been created to honor our academic staff, university staff and faculty.

 

The awards will be given to recognize the personal commitment and efforts of individuals to support a diverse and inclusive workplace, and teach our learners to understand the needs of diverse individuals, families and populations. Awardees will be recognized for their personal commitment and efforts beyond their regular duties and assignments. Criteria for awardees described under each award category below are suggested, but not limited to the following:

  • Academic staff: Individuals in this category may do similar work to the faculty criteria below, and may also serve to promote diversity in administrative capacities. They may promote diversity in outreach activities and in their own work environment.

  • University staff: Individuals in this category will likely be extraordinary proponents of encouraging an open, diverse, respectful, and inclusive learning and working environment in their office, division, lab, or department.

  • Faculty (CT, CHS and tenure tracks): Individuals in this category may provide training dedicated to producing clinicians and scholars who understand the need of diverse individuals, families and specific populations. They may provide innovative teaching in diversity and health disparities. They may also do or support research in health disparities and diversity at many levels, including the potential means to eliminate disparities. They may be strong proponents of diversity and equity in their own divisions and departments.

Applicants from previous years are welcome to reapply with updated materials. Team nominations may be considered.

 

One award in each above category will be presented in the academic year at a school or department-wide celebration. In addition, the awardees will have the opportunity to have lunch and conversation with Dean Robert Golden and receive a plaque to memorialize the award.

 

Application Instructions

  1. A nomination letter, or a letter of support for all self-nominations, is to be provided by an individual familiar with the nominee’s work (not more than 2 pages in length). The nominator or supporter should describe how the individual has stretched him or herself beyond the traditional confines of their job to enhance the SMPH climate.

  2. A statement is to be provided by the nominee describing his or her work and how it reflects the SMPH goal of equity and diversity. The statement should be no more than 2 pages in length.

  3. Completion of the demographic form below.

The review of the applications will be made by the SMPH Faculty and Staff Equity and Diversity Committee.

 

Applications are to be submitted electronically in one pdf file and must be received before midnight on November 16, 2015. Please address the application to the SMPH Equity and Diversity Committee.
