U.S. Students - Eligibility Requirements

U.S. students interested in a clerkship at the University of Wisconsin School of Medicine and Public Health in Madison must be candidates for the MD or DO degree at a school accredited by the Liaison Committee on Medical Education (LCME) or Education Department of the American Osteopathic Association.

 

Students also must have fulfilled the following requirements at the time of application. Students who do not meet these requirements will not be considered for a visiting clerkship.

 

1. Good Standing: Be a student in good standing.

 

2. Final Year of School: Be in the fourth year of a four-year program.

 

3. Pass the USMLE or COMLEX Exam: Passed U.S. Medical Licensing Examination (USMLE) Step 1 or Comprehensive Osteopathic Medical Licensing Examination (COMLEX) USA Level 1 certifying exam.

 

4. Application Fee: Pay a nonrefundable fee of $112.95. A check or money order, made out to the Board of Regents UW-Madison, must accompany application. Note: The fee is waived for students from the Medical College of Wisconsin and MD students applying for neurosurgery and family medicine clerkships.

 

5. Immunization Report: Complete the University of Wisconsin School of Medicine and Public Health Immunization Status Report. Attach copies of your immunization lab reports/titers (see item No. 1 of report) and include this information in your application packet. Do not send copies of your childhood immunization dates or titer summaries from your school. Only the UW immunization status report with copies of your lab reports/titers will be accepted.

 

6. Malpractice Insurance: Have malpractice insurance of $1 million per incident for up to three incidents. No student will be admitted without proof of malpractice insurance. We do not provide it, nor do we know where it can be obtained.

 

7. Personal Health Insurance: Send proof of personal health insurance coverage.

 

8. HIPAA Training: Complete HIPAA training and then print sign and submit the confidentiality agreement.

 

9. CPR Certification: Have CPR certification at the BLS for Healthcare Provider level from the American Heart Association. (A copy of your CPR card verifying certification is required for the duration of University of Wisconsin School of Medicine and Public Health electives.) Red Cross — Professional Rescuer certification also is acceptable.

 

10. Background Information Disclosure: Complete the background information disclosure form. If you answer "yes" to question 3 in section B of the Background Disclosure form ("Have you been discharged from a branch of the US Armed Forces, including any reserve component?") you must submit a copy of your Discharge Papers (DD214).

 

11. Photo: Include a photo with the application (JPG format, less than 1 megabyte; e-mail to visitingstudent@med.uwisc.org; include your name and school in the e-mail).

 

12. Safety and Infection Control Training: Complete Safety and Infection Control (SIC) training.

 

13. Application: Complete the application, with home school's verification and seal affixed.
