Students Attending U.S. Medical and Osteopathic Schools

Eligibility Requirements

When your application has been received and your fee charged, you will be given access to OASIS, the University of Wisconsin School of Medicine and Public Health's online student record system. The instructions for using OASIS as well as the forms you need to upload to meet our requirements will be emailed to you.

The following is required of students who wish to be considered for a visiting student clerkship:

• Immunization Status Form (UW School of Medicine and Public Health form)
• Background Information Disclosure Form
• Safety and Infection Control (SIC)
• CPR Card (BLS for Healthcare Provider level from the American Heart Association)
• Digital Photo (JPG format, less than 1 megabyte)
• HIPAA Training

Note: Only the 2-page printed application will be accepted. All additional paper documents will be discarded.
