Description: Improve perinatal health and safety outcomes for pregnant women and new mothers at risk for intimate partner violence through abuse responsive services, education, prenatal care and advocacy.

Brown County (statewide) awarded $427,068 to "Got Dirt? Garden Initiative."
Description: Improve child health and nutrition by increasing access to and consumption of fruits and vegetables through youth gardens at childcare centers and schools in Wisconsin.

Children's Health Alliance of Wisconsin (statewide) awarded $416,963 to "Keeping Kids Alive in Wisconsin."
Description: Improve child health and safety and prevent child deaths through the development of a statewide death tracking system in Wisconsin.

City of Milwaukee Health Department (Milwaukee County) awarded $426,867 to "Ecocultural Family Interview Project."
Description: Improve birth outcomes in high risk mothers by adapting and piloting the Ecocultural Family Interview, a tool to address family strengths, challenges and gaps in services.

Dane County Department of Human Services (Dane County) awarded $427,486 to "Allied Drive Early Childhood Initiative."
Description: Improve the health of vulnerable young children and their families in Madison's Allied Drive community through the continuation of a comprehensive home visitation program focusing on health and social services and improved access to employment assistance.

Diverse and Resilient, Inc. (Chippewa, Dane, La Crosse, Milwaukee counties) awarded $426,976 to "Reducing Tobacco Use Among LGBT Populations in Wisconsin."
Description: Reduce tobacco use through prevention and smoking cessation strategies for lesbian, gay, bisexual and trans-gender adults in four regions in Wisconsin.

Milwaukee Police Department (Milwaukee County) awarded $424,324 to "Expanded Community Role in the Milwaukee Homicide Review Commission."
Description: Reduce fatal and nonfatal violent injuries and promote healthy and safe neighborhoods through the expansion of violence-prevention activities in partnership with community-based organizations in Milwaukee.

Description: Improve birth outcomes in vulnerable pregnant teens through improved access to prenatal healthcare services in Milwaukee-based school settings.
