The Wisconsin Partnership Program's Community-Academic Partnership Fund awarded the following grants in 2007:

Aurora Sinai Medical Center (Milwaukee County) awarded $362,691 to "Expanding and Sustaining the 'Safe Mom, Safe Baby' Project."


Description: Improve perinatal health and safety outcomes for pregnant women and new mothers at risk for intimate partner violence through abuse responsive services, education, prenatal care and advocacy.

 

Academic Partner: Tina Mason MD, MPH, FACOG — Department of Obstetrics and Gynecology

 

Brown County (statewide) awarded $427,068 to "Got Dirt? Garden Initiative."

 

Description: Improve child health and nutrition by increasing access to and consumption of fruits and vegetables through youth gardens at childcare centers and schools in Wisconsin.


Academic Partner: Aaron Carrel, MD — Department of Pediatrics

 

Children's Health Alliance of Wisconsin (statewide) awarded $416,963 to "Keeping Kids Alive in Wisconsin."

 

Description: Improve child health and safety and prevent child deaths through the development of a statewide death tracking system in Wisconsin.

City of Milwaukee Health Department (Milwaukee County) awarded $426,867 to "Ecocultural Family Interview Project."


Description: Improve birth outcomes in high risk mothers by adapting and piloting the Ecocultural Family Interview, a tool to address family strengths, challenges and gaps in services.

 

Academic Partner: Katherine Magnuson, PhD — Social Work

 

Dane County Department of Human Services (Dane County) awarded $427,486 to "Allied Drive Early Childhood Initiative."

 

Description: Improve the health of vulnerable young children and their families in Madison's Allied Drive community through the continuation of a comprehensive home visitation program focusing on health and social services and improved access to employment assistance.

 

Academic Partner: Roseanne Clark, PhD — Department of Psychiatry

 

Diverse and Resilient, Inc. (Chippewa, Dane, La Crosse, Milwaukee counties) awarded $426,976 to "Reducing Tobacco Use Among LGBT Populations in Wisconsin."


Description: Reduce tobacco use through prevention and smoking cessation strategies for lesbian, gay, bisexual and trans-gender adults in four regions in Wisconsin.

 

Academic Partner: Kathleen Oriel, MD, MS — Department of Family Medicine

 

Milwaukee Police Department (Milwaukee County) awarded $424,324 to "Expanded Community Role in the Milwaukee Homicide Review Commission."


Description: Reduce fatal and nonfatal violent injuries and promote healthy and safe neighborhoods through the expansion of violence-prevention activities in partnership with community-based organizations in Milwaukee.

Partners in Prevention-Rock County, Inc. (Rock County) awarded $416,746 to "Underage Drinking - A Parent Solution."


Description: Reduce underage alcohol use among Edgerton School District adolescents through prevention strategies involving school, family and community.

The Hospice Organization and Pallative Experts (HOPE) of Wisconsin (statewide) awarded $372,407 to "Changing the Culture of Palliative Care in Rural Wisconsin."


Description: Improve patient and family access to healthcare through an active network of palliative care and hospice service providers in rural Wisconsin communities.

The Salvation Army Wisconsin and Upper Michigan (Milwaukee County) awarded $430,612 to "It Takes a Community to Help a Smoker."


Description: Reduce tobacco use through prevention and smoking cessation strategies for residents in the 53212 Milwaukee zip code area.


Academic Partner: Bruce Christiansen, PhD — Internal Medicine

 

Collaboration Development Grants

 

Aurora Psychiatric Hospital (Milwaukee County) awarded $66,972 to "Reducing Youth Substance Abuse through Brief Motivational Interviewing in Schools."


Description: Reduce substance abuse and dependence among Milwaukee area at-risk youth ages 13 to 17 in partnership with five local school districts.

 

Academic Partner: Patricia Kokotailo, MD, MPH — Department of Pediatrics

 

Aurora Sinai Medical Center Midwifery and Wellness Center (Milwaukee) awarded $66,937 to "Group Prenatal Care for Vulnerable Pregnant Teens: Building Self-Efficacy and Social Support."


Description: Improve birth outcomes in vulnerable pregnant teens through improved access to prenatal healthcare services in Milwaukee-based school settings.

 

Academic Partner: Jacquelynn Tillett CNM, ND, FACNM — Department of Obstetrics and Gynecology

 

Benedict Center, Inc. (Milwaukee County) awarded $67,000 to "Reducing Mental Health Treatment Barriers in Adjudicated, Poor, Substance Abusing Women."


Description: Develop strategies that reduce barriers to mental health treatment services in Milwaukee in partnership with affected women.

 

Academic Partner: Ronald Diamond, MD — Department of Psychiatry

 

Community Health Partnership, Inc. (Chippewa, Dunn counties) awarded $66,748 to "Oral Health Improvement for Adults with Developmental Disabilities."


Description: Improve oral health for adults with developmental disabilities in residential and private home settings through screening and referral services, health education and student training.

Couleecap, Inc. (Crawford, La Crosse, Monroe, Vernon counties) awarded $67,000 to "Fluoridation for Healthy Communities."


Description: Continue planning efforts for community-wide collaboration and support for optimal fluoridation levels in four adjacent Wisconsin counties.


Academic Partner: James Terman, MD — Department of Family Medicine

 

Deaf Unity (statewide) awarded $54,443 to "Promoting a Safe and Healthy Deaf Community."

 

Description: Provide domestic violence and sexual assault services for deaf women in Wisconsin through abuse responsive services, education and advocacy.


Academic Partner: Leah Algier, MD

 

House of Peace Community Center (Milwaukee County) awarded $50,840 to "(Kev Noj Qab Haus Huv Ntawm Pojniam Hmoob Lub Neej) Staying Healthy as a Hmong Woman: Building Capacity to Address Cancer Disparities."


Description: Improve the health of Hmong women through the development of prevention strategies that reduce barriers to cancer screening.

 

Academic Partner: Sarah Esmond, MS — Center for the Study of Cultural Diversity in Healthcare

 

Juneau County Health Department (Juneau County) awarded $59,250 to "Creating Healthy Rural Communities."

 

Description: Develop, implement, and sustain a community-wide health improvement process and plan for improving the health of Juneau County residents.


Academic Partner: Barbara Duerst, RN, MS — Department of Population Health Sciences

 

Marshfield Clinic Research Foundation: A Division of Marshfield Clinic (Barron County) awarded $67,000 to "Uniting a County."


Description: Reduce alcohol and other drug abuse among 12 to 17-year-old adolescents in Barron County through a coalition-directed community action plan.


Academic Partner: John Olson, MD, FACP — Department of Medicine

 

West Central Wisconsin Community Action Agency, Inc. (Barron County) awarded $59,864 to "Family Table Project."


Description: Improve the nutrition of low-income Barron County residents through a family-centered and shared meal preparation program using locally grown whole foods.

 

Academic Partner: Kirstin Siemering, DrPH, RD — Academic Affairs

 

Wisconsin Department of Health and Family Services (statewide) awarded $66,873 to "Healthiest WI 2020: A Partnership Plan to Improve the Health and Safety of the Public."


Description: Develop the next ten-year public health plan to guide and direct the shared vision of healthy and safe Wisconsin communities.
