Wisconsin Partnership Program Education and Research Committee Grantee Forms

The memorandum of understanding between the grantee and the Wisconsin Partnership Program Education and Research Committee (PERC) requires as a condition of the award progress reports, financial status reports and a final report.

Financial status reports are required no later than 30 days after the end of each six-month period, progress reports no later than 30 days after the end of each 12-month period and a final report no later than 90 days after the end of the project.

The forms below are available for Medical Education and Research Committee grantees, including those funded as part of the New Investigator Program and Collaborative Health Sciences Program.
