Application Instructions - Step A
Use the e-Payment link to charge the nonrefundable application fee of $125. There are no additional tuition fees. Note: The fee is waived for students from the Medical College of Wisconsin and MD students applying for neurosurgery clerkships.
Your application including uploaded documents must be complete at least four weeks before your clerkship begins at the University of Wisconsin School of Medicine and Public Health in Madison.
1. Fill out the Visiting Student Account form (OASIS).
2. Proceed to application.
3. Select the year.
4. Fill out personal information and save it.
5. Click on the "Step Two: Select Schedule" button.
6. Click on "Add Course."
7. Choose "Department" and click "Search."
8. View catalog information by clicking on "Course Title."
9. Add a course to schedule by choosing "Number of Weeks" and "Start Date." (If a course is open at the time you complete your application, you will be able to add it. If it's full, you'll be notified immediately. Please note: This is only a list of courses you'd like to take. You will be notified when your application is complete if the courses you requested are still available.)
10. Continue adding, dropping and renumbering clerkships until your application is complete.
11. Click on the "Print Application" button.
12. Print both pages of application.
13. Have your school complete page two and affix the school seal.
14. Mail your application to:

Note: Paper applications will only be reviewed if the $125 nonrefundable fee has been paid.
Students Attending U.S. Medical and Osteopathic Schools: Only the 2-page printed application will be accepted. All additional paper documents will be discarded.
Students Attending International and Caribbean Medical Schools Regardless of Citizenship: Only the 2-page printed application and your attending's evaluation of your performance on a clinical clerkship in the U.S. will be accepted. All additional paper documents will be discarded.
