Sept. 15, 2019 - UW-Madison Doctor of Physical Therapy Program golf outing

When: registration at 11 a.m., tee off at noon
Where: Yahara Hills Golf Course, 6701 US Highway 12 and 18, Madison, WI, 53718
Pricing: $95 for a single golfer, $350 for a foursome, $65 for a student golfer. Each golfer receives 18 holes of golf, dinner and a beverage ticket. A dinner-only option is available for $25. Proceeds go toward student professional development and additional experiences to enhance the DPT education.
Sponsorships: Silver Level $150 includes signage on tee box and website advertisement; Gold Sponsor: $500 signage at registration, three tee boxes, website advertisement, and two dinner tickets
